• Homestead Exemption Form

IMPORTANT NOTE TO FIRST-TIME APPLICANTS: Do not mail in the completed Homestead Exemption Form. Bring your application in person. Only one signature is required for joint owners who are married with the same last name. However, all documentation listed below must be brought with you. All signatures are necessary if owners who occupy home have different last names.
THE FOLLOWING INFORMATION IS REQUIRED:

1) RECORDED WARRANTY DEED OR LAST TAX BILL
2) FLORIDA DRIVER'S LICENSE: All joint owners who reside on property need Florida Drivers Licenses with current address. ''Valid in Florida Only'' (VIFO) licenses are NOT sufficient.
3) FLORIDA VEHICLE REGISTRATIONS: All vehicles owned by applicants must have resident Florida registrations with current address. Leased vehicles and non resident registrations CAN NOT be accepted.
4) SOCIAL SECURITY NUMBERS: Social security numbers are required for all owners listed on deed.
5) *DECLARATION OF DOMICILE OR FLORIDA VOTER'S REGISTRATION CARD: If an owner does not have a car registered in his/her name, one of these documents will be required as proof of residency.
NOTE: MEMBERS OF THE ARMED FORCES ARE NOT EXEMPT FROM THESE REQUIREMENTS.

• Physician's Certification of Total & Permanent Disability

IMPORTANT TO FIRST-TIME APPLICANTS: DO NOT MAIL THE COMPLETED CERTIFICATES. Bring two certificates to our office in person along with a notorized copyof a completed ''Statement of Gross Income Form'' (see below)


• Statement of Gross Income (Total and Permanent Disability Applicants)

Notorization of this form is required.

• Sworn Statement of Adjusted Gross Income Of Household and Return
• (Senior Citizen Exemption)

INPORTANT NOTE FOR FIRST-TIME APPLICANTS : Do not mail the completed form. Bring the form to our office in personalong with proof of birth and supporting documents (Prior year's federal income tax return with W-2 form for all persons in the household). For those who are no longer required to file a Federal Income Tax Return, all Form 1099 for Social Security, retirement accounts, bank accounts, etc. must be presented.
